Whether an employee or job candidate, you will need to bring an eScreen ePassport or a completed Employer Authorization for Treatment Form, signed by the company representative requesting the service, as well as indicating the service(s) to be performed or an eScreen ePassport code. They must also provide a photo ID (driver’s license, passport, military ID, employment badge or student identification card). Appointments are encouraged, but walk-ins are welcome.

A parent or legal guardian should accompany a minor. If you are not the legal guardian, you will need a notarized statement signed by a parent or the legal guardian authorizing you to give permission for services and billing, if appropriate. This procedure is no different from treating minors with urgent care needs.
Unfortunately, no. Due to the nature of certain services—for example, drug testing, only the patient and staff are allowed in the testing area. We are also keeping visitors at a minimum during COVID-19.

We encourage making an appointment so that your wait time will be potentially less. For DOT Physicals, it is important to make an appointment due to availability of the clinical provider. Walk-ins for other services are welcome.
